Replacing on off knob

I have replaced items 5 and 6 on my old Aqualasia Classic and need to know how to secure the new item 6 (on off knob) onto the spindle - it just comes off and is not attached - it sits on the threaded section of item 5 but comes off far too easily when turning the shower on

There should be a small ‘O’ ring on the spindle of the valve control this causes an interference fit for the on/off knob
