It may be a faulty Temperature control valve however I would make sure that all the filters are clean and clear first.

Is this on a combi boiler?

Also it may be worth checking all the filters, the other possibility is a faulty mixing valve, as the valve is fairly expensive it may be worth stripping the cartridge out of the valve and sending us a photo of the cartridge as we may be able to source the cartridge on it’s own.
