Pump runs, no flow until showerhead below pump

I have an aqualisa aquastream post 1997,

The pump runs but no water is pumped until I take the showerhead off the wall and lower into the bath.

Howvere, the water continues to flow then when I return the showerhead to the wall bracket.

Is there an air lock , or is there a water pressure switch not opening to prime the pump, the priming has to be done manually by letting the water flow by gravity.

The water pressure from the tanks should keep the water in the pump area of the shower.
You may have a leak around the cartridge or coming into the shower which would need fixed.
