No water or water doesn't stop

On your advice I recently ordered a cart 5, which arrived very rapidly (many thanks) and was the right cartridge for our shower. However, when I installed it, no water came through. I reinstalled it, and this time the water came through when I turned it on but I couldn't turn it off.

The cartridge fits in a kind of sleeve set into the wall of the shower; and the knurled nut at the end of the cartridge fits into a recess, which seems to be ridged. I assume this is meant to hold the nut fixed while the valves open and close (is that right?). Is it possible that the problem is that the recess has become worn and no longer grips the nut? If so, does that mean replacing the whole fitting?

As far as I can tell, I installed it exactly as described, but it still doesn't work.

If I've understood the working correctly, once the cartridge is seated fully in position (before putting on the retaining ring), it should be possible to turn it only through 90 degrees. If so, that's the problem: I can keep turning the cartridge without it being stopped after 90 degrees. Presumably this means that the nut at the end which controls the ceramic disc valve and flow is not being held in position. Does that sound possible?
