Shower won't turn off

I have this cartridge (brand new) installed in a concealed shower. The external control to turn the shower off is screwed to a plastic ring which as far as I can make out doesn't actually turn anything to control the flow. The problem is that you have to try several times in different ways to get the shower to turn off. The flow also changes (with the control off) when you switch form hot to cold. I'm an experienced plumber but have never seen anything like this! Any ideas on whats wrong and how to fix would be very much appreciated.

This cartridge is used in a few showers and you don’t mention which shower you are working on.

Can you send us a photo of the shower or can you see it on our web site?
