Can’t turn off the water

I replaced the seals on my cartridge about 5 months ago which solved a drip. Recently the drip came back and I’ve had to turn the on/off lever further and further to stop the water flow. Today the lever is just spinning and can’t be turned off. I dismantled the cartridge and there doesn’t appear to be anything to stop the water flow control from spinning. Could I have I stripped the threads that stops flow by turning it off too tightly?

There is a circlip that holds the on/off part to the top of the large brass nut, and if this has snapped or is out of place then the symptoms you described could happen.
