Leaking shower

Hi, I think this is the type of shower I have, the shower was fitted when the house was built in 2004, it has a chrome finish on all parts with a large on-off tap at the front of the unit, with the hot/cold temperature control a large tear-shaped knob in front of that. It has developed a leak from the head, running constantly, and I believe a need to purchase a seal kit or similar. I'd be very grateful if you could advise which part I require. Many thanks
