Valve Leak
There were two problems with this in the past if the valve started to seize up then forcing it to turn could force it past the clip.
However if you have one of the older valves when servicing it you may have to push the spindle through with one of your thumbs from the inside.
What happens is when it is greased and pushed in to place a pocket of air forms at the end of the spindle and because of the grease there is no place for the air to escape, by pushing it up you should be able to compress the air enough to get the clip on, when the valve is used this soon sorts its self out.
The later models have a tiny hole bored in the top of the cartridge to allow the air to escape.
