Having trouble removing the lever assembly from the head

I need to replace the whole of the lever assembly as the thread of the screw fitting which kept the porcelain handle in place is worn. I am having trouble removing the remaining lever assembly from the head. Am I right in thinking I should just be able to pull it off? It seems stuck fast. I've tried WD-40 and soaking it in hot water and white vinegar but to no avail. Is it possible to lever it off if I wedge a flat head screwdriver in between? Any suggestions would be greatly appreciated!

Yes the handle piece sometimes can stick onto the cartridge, and as you have tried every other thing I would have suggested, it maybe time for the flat screw drivers.
